Medicine is where
patients are admitted to when they cannot be discharged from the ER. They usually require
additional treatment that can take from days to weeks (sometimes even longer). Those that
respond to the treatment are, at some point in time, seen by a social worker to discuss
discharge plans. For some our involvement is minimal, just relaying information about community resources that the patient or family MAY require. For others it is more extensive including contacting resources that the patient or family WILL require. The list of services and resources that a social worker is familiar with and may have to access on behalf of the patient or family is extensive. I have started to compile a list of local resources but it is going to take time. For now check out the social work links that I have found. More will be added as they are found.
